Definitely get your visa before you go. You can get it on the internet. You just need to take two printed copies of the page they send you (black and white is fine). You hand in one on entry and one on departure.

We went from Bangkok. Don't trust anyone! Don't go on one of the shuttles - they can take a long ass time! You can catch the train to the Chatuchak market area (I don't remember the station name, though, sorry) and catch a taxi to the train station (it's pretty close to there). It will cost you more, but it is reliable.

When you get to the border town, you have to catch a tuk-tuk to the actual crossing. Be careful at the crossing - there are really sh*tty touts who will try get you to take dodgy taxis. It should cost you about $45 or $50 to get to Siem Reap.

The road is something you won't ever forget.

I got a 3 day pass and on the 3rd day felt like I was going just to use my pass. Don't get me wrong, I'd love to go back but 3 days back to back gives you the 'templed out' feeling. You can get 1 day passes, but getting 3 1 day passes would be more expensive than getting a 3 day pass...pity really.

Tuk tuk's are cheap and you can hire them for a day. A lot depends on your budget, length of stay in the region and what you want to do.
