So, I'm filling out the Application for Visa and I'm coming across some questions that I'm not sure what to put.

On question 10 and 11 regarding my passport it asks:
10: "Place of Issue"
11: "Issuing/Authority"

Now, I'm probably thinking to hard about this but for 10 I don't know if I put Chicago, Illinois, or USA. And I have no idea what to put for 11.

For question 16,17, and 18 they want me to list my occupation, right. So I would write down social worker?

19 asks "Purpose of Entry (Explain fully)." Should I just put "Teaching"?

Last question, for number 27 it asks :Who will pay for your trip?" Since I'm buying the ticket myself and then being reimbursed once I get there do I write down "Self?"

Please, help me with this. I'm just worried that I'm gonna do it wrong and then have to do it all over again which will delay me going. Any input would be greatly appreciated.

10. Did you get it issued in Chicago? Then put Chicago.
11. It says right there in your passport under "authority"

16-18. What is your job? You put that.

19. Is this for a work visa or travel visa? If it is only for a tourist visa and you do not have a work visa do NOT put teaching. If it is for a work visa and you will be teaching, put "teaching".
