The line on the visa means it is no longer valid (you left during the validity period) and cannot be used again (a new visa would be required).

In regards to the question on the form, answer, "yes," and when asked (or just add it next to the question) visa canceled when I left Korea (quit my job)."

When your visa expires after one year from getting it, you shouldn't have any problems what so ever. Immigration doesn't care about broken contracts, it only cares about your old E2 visa being expired and being a criminal (i.e. breaking Korean laws or laws in other countries). Make sure you teach at a public school when you come back.

I believe I answered yes where it asked if it was cancelled or revoked. They didn't really ask me about it. They have that sheet when you go in for the visa interview, but I interviewed with my girlfriend and the guy at the consolate was too busy drooling all over her to bother looking at my stuff. I was expecting an interegation and Ill I got was how lucky I am to be dating a girl that looks like a movie star or something. It was pretty wierd. Compared to my first visa interview that took all of five minutes, this one took almost 50 and it was all about our relationship. "What do you love about her?" etc. Kind of awkward, but I didn't care because it was a welcome distraction. There is no black line on my visa, it says CANCELED in big red letters.
