90 day entry stamp = no.
30 day entry stamp = yes (go to immigration).

30 day tourist extension from an E2 = no.

C3 = not usually.

you can get a lot of info off their website. i'm too lazy to look up the address, sorry, but I've extended my visa online after paying a small fee and just printed out a paper that I gave to immigration when I left.
