The police department does not do fingerprinting here in Hawaii, so I have to use an outside service. Cost is $20, so about the same, but is this okay? Also is it okay to just use the print out piece of paper from the FBI website and not a piece of cardboard.

The last question, need clarification on these fields on the fingerprinting form
your no
FBI no.
Armed Forces no.
Miscellaneous no.

I have no idea what those fields mean

Any help would be greatly appreciated.

There have been some folks on this forum who have said that the online printout has worked. I can't say "yay" or "nay" about that.
Keep in mind though, that the fingerprints are for an FBI criminal background check; and the FBI site states that you need the official cardboard form.
I used the official form, and suggest you do too, if possible.
Chances are the agency you are using will have the official forms--tell them that it's for an FBI CBC.
I don't know which agency you're going through, but this one seemed to me to be the most official [email protected]
This site explains how to fill out the form.
As far as those other fields; don't worry about them cuz they don't apply to you.

I had mine done at my hometown police station on a laptop with a reader device that printed it on card stock identical to the FBI card everyone are fresh out of. I got 3 copies (send 2) so one can be kept for scanning and printing if I need to send in for another FBI background in the future. You might ask if the service can produce them on their cardstock with a printer.

I did mine myself, at home, using the FBI form printed on A4 paper.

No troubles.

I sent in THREE copies just to be sure that they could do it, but there was no trouble at all.
