I am looking at the website for "Authentications and Apostilles"
http://travel.state.gov/content/travel/en/legal-considerations/judicial/authentication-of-documents.html

And under the "Authentications" section there is a link to follow to download a form for an "Authentication"
But under the "Apostilles" section, there is no link to follow for any form to order an "Apostille"

I downloaded the form for "Authentication" and there is no mention of Apostille anywhere on that form, so it doesn't seem to be the right form.
That form is DS-4194 : U.S. Department of State REQUEST FOR AUTHENTICATIONS SERVICE

So, if anyone has successfully navigated that Apostille website recently, can you tell me where to get the form for applying for the Apostille. Or, if using the form for Authentication, how to specify that it is an Apostille you are seeking.

Nevermind. I found the info buried deep in a few submenus. It's the same form for Apostille.
