I have worked in a women's university for 2 years and this has never been an issue and I have never spoken to a teacher where it is an issue. I have had my students pop their hands up and ask to go to the bathroom. I have told them that if they need to go-just go. No one, to my knowledge, has taken advantage of this. I have few students ever leave my classroom, if they do, I trust that they have their reasons.

High School may be different, at least it certainly sounds like it. If my students all ran out together then it would quickly become an issue.

Schoolgirls to Have One Day Off a Month
Female students will soon be able to take one day off every month during their monthly period, according to the Ministry of Education and Human Resources Development. The ministry announced the plan as part of protecting female students' health rights and reproductive health. It will implement the policy from March as a pilot program in four schools, two coed middle and high schools and two girls' middle and high schools. The authorities will decide whether the policy is to be fully implemented, depending on the result of the pilot program in schools.

Under the new policy, schoolgirls will be allowed to skip class as long as they provide a note from their parents confirming their condition. Instead, they will be allowed to substitute the missing tests as scores from the most recently taken exam.
