I've got a class with a girl who has been studying in Japan at an internation school. Practically fluent in both English and Japanese. Yes, fluent IS a strong word, but if she understands idioms and sarcasm I would definitely call that fluent. I consider fluency to encompass the ability to pick up the subtleties of a 2nd languages humor.
