Hi,
I've applied for a public school job and I've been given a medical form to fill out. I'm used to being asked personal questions, like age, weight, etc., but some of these questions on this form seem really personal. Here's the form:
Medical Form (Self Evaluation)
To the applicant : Please provide correct information for the following questions. Any omission or false information will delay processing of your application. All applicants will be required to submit an Official Medical Report from a practicing physician before the interview.
1. When and for what reason did you last consult a physician?
2. Have you had any serious ailment, injuries or diseases in the past five years?
Yes No If yes, please explain.
3. Have you been hospitalized in the past two years?
Yes No If yes, please explain.
4. Have you ever been treated by a doctor for any mental, emotional, or nervous disorder?
Yes No If yes, please explain and attach a report from your doctor.
5. Have you ever been addicted to any substance? Yes No If yes, please explain.
6. Do you have any allergies? Yes No If yes, please list them.
7. Are you taking any prescribed medication?
Yes No If yes, please list and explain why.
8. Are you on a special diet? Yes No If yes, please explain in details.
The answers I have given are true and correct to the best of my knowledge.

Most actually require that you go and get a full medical done sometime early in the contract, and give the results to the employer. |

They don't seem to have the same feelings about privacy in Korea as we do. People on the street will ask you how old you are, whether you're married, or even how much you weigh. |

In my 9yrs here I have only done a medical once that was work related. One year my school said they would pay for all the teachers to go have one done. I figured why not, everyone else was doing it.
Man what a joke that was. It made me laugh they considered the tests they did truly worthwhile. |

If you are sick, we don't want to hire you. Welcome to Korea. Yes, these are normal questions. Lie if you want. In my opinion, some of that is none of their darned business. I'm sure many Koreans lie about this stuff, too.
You will find that every other hospital is different in how they carry out the medical exam. Some are very thorough. Some listen to your heart, make you pee in their coffee cup, and kick you out the door. |

Also, be prepared for the form to be displayed or discussed openly, with no regard to privacy at all.
I was thrilled one day to see my medical report lying on my Supervisor's desk, with all details of my weight, chest circumference etc visible to anyone who happened to walk past. |
