It means they are more sensitive to pain.

In this case, threshold means level. A lower pain threshold means that you have a lesser ability to stand pain

The word lowered was used in your snippet to indicate the decrease in pain threshold. The word lower would be more likely to be used for comparison to the previous state. Here, probably either word would work, but the author may have chosen lowered to indicate the cause-and-effect relationship.
