A. is better:
I'd get it for you if only I could remember who last borrowed it.
'on the condition that' is better used in situations that are negotiable. EG 'I'll drive Fred to the airport on the condition that he pays for petrol'.
In the first sentence nobody really has any control over whether the subject can remember or not.
