Questions like this can be grindingly difficult to answer because there are dozens upon dozens of different situations where we use these words. I'm not going to try to sift through every single one of them, searching for slightly differing nuances of meaning. Generally, they are synonymous. Swap tends to be somewhat less formal than exchange. If you have a certain sentence or specific context in mind, that would be swell.
