what happened refers to an event that is already past:
A: Sorry I'm late.
B: What happened?
A: My car had a flat tire.
what's happening (sometimes also "what's up") is an informal way of greeting someone, of asking how they are doing:
A: Hey, Bill, I haven't seen you for a long time. What's happening / What's up?
B: Oh, not much. I've been busy coaching basketball at the park district on Saturdays.
