Hi. I just discovered the site and saw your message.

By now, I'm sure you're either registered at the site or have given up. If you still need translation help and are still around, please post a followup and I'll be glad to help.

Hi, saaya.

It looks as though the original poster is no longer around. But since you appear to be interested in the translations, here's my best attempt:

1.이 름 - name
2.주 소 - address
3.상품수령지 - Not sure about this one - Maybe "product recipient's location"?
4.주 소 - address (again))
5.나머지주소- remainder of address
6.전화번호 - phone number
7.서류파일 - document file
8.저희 '쁘띠마르샹' 에서 제공하는 다양한 정보를 받으시겠어요? - Will you receive the various information offered by ? (I have no idea what 쁘띠마르샹 is. A company name, maybe? A product? It doesn't look like Korean.)
9.예 - yes
10. 아니오 - no

Anybody else?
