My mother will take out a traveler insurance on the week on Monday.

Anything wrong with the sentence?

Hmmm, I'm not sure if there is anything wrong with it, but I would say "travel insurance" or "traveler's insurance", drop the "a".
Not sure about the preposition before "week". Do you mean "for the week"? As in, she will buy the insurance on Monday, and it will cover her for the whole week.

Different ways to rephrase:
My mother will take out traveler's insurance for the week on Monday.

Monday, My mother will take out traveler's insurance for the week.

My mother on Monday will take out traveler's insurance for the week.

My mother will take out traveler's insurance on Monday for the week.

alternate verbs for will take out:
will buy
will get
is getting
plans to get
plans on getting
