On a DVD I heard someone called flaky. What does this mean?

Someone who is flaky (a flake) is offbeat. He/she is harmless, but he'll tell you he'll meet you at 3:00, and then not show up because he forgot all about it. He probably has a short attention span, thinks odd thoughts, has odd habits, etc. Sometimes they are lovable, sometimes exasperating.

How about "freak"? If someone is flaky, then he must be a freak, right?

Not necessarily.
A freak (a person who is freaky) is someone who is strange. A flake (a person who is flaky) is someone who is irresponsible and unreliable. They don't have to overlap, i.e. a person may seem to be a freak, but they might still be trusted to do what they said they will.
