Does the above sentence mean 'On his stomach, face being thrust into the dirt, he tried to conceal his neck and ears with his green flight gloves.' or 'On his stomach, face thrust into the dirt, and he tried to conceal his neck and ears with his green flight gloves.'?

As I read it, his face has been thrust into the dirt and the past participle thrust is being used here as an adjective, describing how he appears.

I might also say,

"On his stomach, his face having been thrust into the dirt, he tried to conceal his neck and ears with his green flight gloves."
