Please be advised that a Notification of Third Offic Action issued May 12, 2006 is received on May 22, 2006.
I would suggest editing it to:
"Please be advised that the Notification of Third Office Action, issued May 12, 2006, was received May 22, 2006."
The changes: Changing "a Notification" to "the Notification" (assuming that is the title of the document); spelling of "office"; adding punctuation for clarity; getting rid of "on" for the second date. You have "May 12" and "on May 22." If you want to use "on," that's fine, but I would use it for both dates.
