If a prophecy doesn't come true, is it OK to say "The prophecy failed."?
If not, what kind of verbs can be used instead of "failed"?

I should think 'failed' is good enough.
The prophecy went wrong/didn't come true. |

hey there!
i would recommend using "the prophecy was inaccurate". prophecies are more like guesses: they don't have an expected standard of success, and sometimes they are partially right or ambiguous. so they don't usually get labelled as a pass or fail, but are graded more in terms of their accuracy.
