It means you have been immunised against smallpox. It is not an antibiotic, but a harmless form of the smallpox virus that makes your own immune system capable of killing the live form of the virus.

You are thinking about antibodies and the immune response.

When the antigen (invading organism) is introduced into the person's body, the immune system fights it off (the immune response) and creates antibodies (not antibiotics) to destroy or at least neutralize the antigen. The antibodies recognize the antigen whenever it is introduced later, and they quickly take care of the intruding virus or bacteria.

If you received a flu shot last year, you got a harmless form of the virus. Your immune system developed antibodies against that particular virus, so if you were exposed to the real thing later, you did not fall ill, or at least not for long. If you did not receive the shot but were exposed to the flu virus, you developed the same antibodies, but you had to suffer through 7-10 days' worth of unpleasantness first.

Antibiotics are drugs that kill bacteria, while antibodies are developed by the body naturally. Some antibiotics are specific to certain bacteria; others are broad spectrum antibiotics, killing off many kinds of bacteria.
