For how long has she been on this drug?
Your second choice is not correct, and the only edit I can think of ends up the same as your first sentence.
"Is" does not work in these sentences beacause the action includes the past as well as the present. |

Hi,
I'd like to add - that although grammatically correct - in common speech, we generally do not use the "for" at the beginning of sentence. We commonly say "How long has she been on this drug?"
