Have an anaesthetic

He's going to have an operation and will have to have an anaesthetic

What do you understand by an anaesthetic. Does it refer to the actual injection or gas, in the same way that have an aspirin, an aspirin refers to the actual pill? Or does it refer to the action, like a haircut in have a haircut?
