So apparently I got evaluated by my students today and I didn't do so hot. I got a 3, with 1 being the best and 5 being the worst. So the POE had me write a letter explaining what I would do to improve my teaching methods.

Is this because I scored poorly or is this something everyone has to do at the end of a semester? More importantly what does this tell me about my job.

That type of feedback has nothing based upon your teaching skills. There is no constructive feedback, nothing based upon what you mentioned. If I turned round and said "You scored 3 out of 5, you're a bad teacher so write me letter detailing what you would do to improve" that would be useless information.
